Jury Questionnaire Information
According to the law, you are required to answer the following questions on the Questionnaire and return it within 14 days. Failure to comply with this notice within 14 days or any misstatement made will subject you to penalty for Contempt of Court.
If you are “qualified” to serve as a juror, your name may be drawn by lot (by chance) for jury service at a later date, at which time you will receive a formal Jury Summons specifying the date, time, and the Court before which you are to appear.
There are no “Exemptions” from jury duty. Persons are excused only if they have served on Jury Duty within the past one (1) year time period, or have a medical condition which would prevent them from serving. If you believe you have a medical condition that would prohibit jury service, you must also attach a detailed statement from your doctor that describes the condition and the reasons you would be unable to serve. Detailed statements from your doctor can also be faxed to the Jury Commission Office at 217-277-2072 if you filled out the online questionnaire.
